Question
I have a 5th wheel camper and I want to make a concrete pad for the camper to rest on. The fully loaded camper weights 12,400 pounds. The pad will be 33 feet long and 13 foot wide. How thick should  the concrete be and what type of concrete should be used ?

Answer
AS long as you keep in mind..the weight is on 5 points.  4 wheels and the tongue.  Assume the wheels take up 6 sq ft total.   That area needs to be at #3500 psi.  That is a 6.5 sack mix at 4" with steel reinforcement.  If you add depth, you can reduce the steel and the cement slightly.  5" a full depth, and 6 sack mix...with mesh instead of rebar..that should do.  It needs to cure 10 days slowly to prevent fracture.  I would suggest a minor footing at the entrance to the slab.
