Hello!  Thanks for being an expert to answer my question!

I am a police officer in North Carolina.  I am taking an online Law Enforcement Leadership Course from an online college and our current assignment is to write an internal affairs report.  We were given a set of facts and circumstances and we have to write the official report.  

My question is: What is the format used to write the report.  I tried to find sample reports online, but can't seem to locate anything but generic stuff.  How should I format my report, as an internal affairs investigator would.

I am so sorry for the delay in responding, somehow this question fell through the cracks.
I can tell you that when it comes to federal law enforcement reports, there is no specific format, many divisions have their own that they may use and will vary greatly on the type of report you are writing. As a rule of thumb use your standard academy format of who, when, where, when, and how.
Now if you need specific lay out for specific reports i.e. intel, investigative, internal etc... please let me know. For the most part here are the titles I used, which is a general memo format with titles;

XXXXX (Type of Report) Report

RE: (Be specific)

Use bullet titles pertinent to your report. Ensure to add any labels (Classified, Secret, Top Secret, Internal Memo etc...) and close with a NOTE which you can put your personal perspective and thoughts on the report.
