Might I suggest that if you have a logo or something that represents his job, you make a copy of it and trace or draw it on graph paper, using one square per stitch.
after makeing the logo the center design, you can find a border to put around it. there are many designs that you can find online for free.
I did a search for a logo, but did not find any, partly because I have no idea what a psycologist logo might look like.
