Question
I would like a second, or may I say third opinion regarding two roots in need of extraction. What my situation is that I have two front teeth that no longer have any crown to them only the roots that are still visable and exposed through my gums. I am having a patial dental plate made since the teeth are not eligable for caps. I was told at first the roots could remain there for as long as they remained healthy. Now I am being told by a new dentist they need to come out period. Can I get an honest answer to whether they need to come out or is it in my best interest or the Dr.'s best interest that they be removed?

Dear David,

Most dentists do recommend removing roots that are retained with out the clinical crowns,especially prior to the fabrication of a partial denture. If you are planning to have your partial denture to be a permanent way to replace your missing teeth, than taking out the roots is certainly good idea!

Some patients are contemplating having implants placed, but do not have the money .  Leaving the roots in place sometimes can maintain the bone so that implants can be placed at a later time and  I sometimes choose this option especially if the roots have had a previously done root canal.

The truth is that if they are not infected at the apex yet, it is not an emergency.  If the teeth have had prior root canals, they are less likely to get infected rapidly. On the other hand, if no root canals are present, the risk for infection is higher.
