Question
Hi, I have had a root canal and an apicoectomy on #14 10 years ago.  I am now having pain and the endodontist would like to redo the apio on the mesial buccal root where there appears to be infection.  I had a Reclast infusion in september of 2008 so there is concern over osteonecrosis.  Can you give any advice on how to minimize my risks. Mary - I am sorry to say that no one is able to truly predict if osteonecrosis will occur after dental treatment.  If you truly have an infection there really is no choice.  You need it treated.  Even the best surgery can cause a problem, but although a lot is written about the osteonecrosis in women taking osteoporotic medications, only a small percentage fortunately do develop ostenecrosis.  If you have to have that tooth treated, you really don't have a choice.  You can leave the infection brewing there alone, but problems can develop from that also.  I wish I could give you a definite answer, but there really isn't one.  I wish you well and hope it all goes well; chances are it will.
