Dear Steven,

I have suffered with severe tmj pain, clicking and swelling for three years following chiropractic adjustments to my jaw. My jaw had a aways clicked upon opening and closing, but I have never had any pain. But following adjustments I am now left with this problem. I have had an MRI scan with a view to surgery on the disc, but it was found that the disc was still functioning. I am now having pain, swelling and discomfort from a wisdom tooth. My dentist thinks it should be removed but I am very frightened at the possibility of further complications/ pain with my tmj. Is it possible that that from the op that is could further reduce the function of the joint, or in your experience that the removal of the tooth would reduce the pressure in my mouth and possibly reduce symtoms.

Stretching the mouth open wide for surgical access during removal of wisdom teeth has the potential to strain joints that have sustained prior damage.  Your oral surgeon should be made aware of the pre-existing TMJ condition.  It is best if you are sedated during the visit, as this relaxes the jaw muscles and reduces strain on the joints.  If the wisdom tooth is infected (as it sounds like it probably is), removal may be a must.  Further delay only increases the risk of additional problems.

It will take 4-8 weeks after removal of the wisdom teeth for healing to be completed.  It is normal for there is be jaw stiffness, jaw soreness (especially when eating), and there may be a limited range of jaw opening during this period.  Should the pain persist beyond the normal healing period, I suggest you seek professional help from a dentist with advanced training in TMJ disorders.
