Question
Dear Dr Fisher,

I am experiencing red ‘spot like’ bumps on my legs and buttocks. I have had these as long as I can remember. They are very numerous and never seem to resolve. They are mainly small and not too much of a worry for me.

However, over the past year or so some have been getting larger and look like acne (with a yellow head). Or sometimes the spots are just very large and red with no pus but look rather 'angry'. A few of these will happen each month recently. When these heal, they seem to leave a small scar which does not heal fully. A little indented and very red with hyper pigmentation. However, this hyper pigmentation does not seem to resolve – even after a year of having these inflamed like spots the area is still red. Because this is causing me to worry, is this a condition worth showing a dermatologist? I would, of course, not like any marks on my legs from these spots. Are there treatment options to help control this?

One final thing I should meantion which may be of interest, I have taken a course of Isotretinoin which was great for my acne but curiously it did nothing for this condition.

These lesions you describe could be keratosis pilaris which is a genetic disease and is treated with exfoliants ie Salkera, Retin A or others.  The lesions on your buttocks could be acne or keratosis pilaris.  I recommend that you visit you local dermatologist to get a proper diagnosis.
