Question I have a situation were a U.S. distributor of foreign wines is requesting a "Custom Agreement" for the sates of New York and New Jersey. What is this and how may I obtain a draft agreement for this purpose.

I don't know what you mean, because what you've defined does not make sense as a matter of procedure- Are you ordering a full container load of wine-??? from...???
"Custom made Agency agreement " or you may be implying matters about " Custom entry conditions" via the issue of an import permit or license- which must also include a Certificate of origin-
But if i continue on these paths - everthing becomes complex-
Tell me on a follow up the Following-
(1) You are an intermediary buying and reselling for Commissions?- or are you actually buying and taking possession of goods yourself-??? for reselling as a Retailer..? Wholesaler?
When I know that answer to above question ??? then I can indeed give you a direct answer-
In any case the following minimum will apply to both situations-
Offer, Contract, Payment, Delivery- until you get to close to final contract , there is nothing to consider in matters of importation- Offer, contract period usually take 30 days or more to complete-
The supplier is responsible for all matters or export, you are responsible for all matters of import including paying freight, tariffs, duties , and import taxes-in a CIP deal(CIF), thus you wll need to contact cutoms eventually in regards to YOUR Federal and statutory state import obligations as "Buyer"
The only way I can see that the supplier would ask for such information is that you have ordered an "Arrival" (EXSHIP) delivery application-Intermediaries/brokers cannot deal in "EXSHIP" deliveries-
