I want to go back to my maiden name i have been divorced almost 4 yrs. do i just take my birth cert. to the dmv and social security office then can notify any creditors or whoever of my name change?

Dear Jill,
I am not an attorney.  If a return to your maiden name was in your divorce decree, then simply go into all the offices and get the paper work started.  If it was not included in your divorce decree, then I think, in many states, it's a legal name change procedure.  You might call the clerk at the courthouse in your county.  They won't give legal advice, but often they can direct you.
