Dear Dr. Saleeby,

I am a 53yo woman with long term health issues dealing with gastric reflux, intestinal permeability and some food sensitivities.  I have been taking medicine for many years for the gastric reflux, currently taking ranitidine (generic Zantac) 150mg 3 times a day.  

I have recently become interested in a vitamin regime, assuming that the medication I take would have some effect on my body's ability to fully harvest nutrients from the foods that I eat.  

I just read and took copious notes from the book, Optimal Digestive Health, c1999, 2005, edited by Trent Nichols, MD and Nancy Faass.  I was first referred to Dr. Nichols' office years ago and diagnosed with leaky gut and inflammatory colitis.  He has since retired.  

With the books recommendations in my hand I went vitamin- and mineral-supplement shopping.  I have B-Complex vitamins in tablet form, plus a sublingual form with B12, B6 and Folic Acid.  I also have 2 different bottles of a Calcium & Magnesium formula, one of which also includes Zinc.  I plan to take one formula one day, and the other the next. One of the Calcium-Magnesium formulas also contains Vitamin D.  

I also have Fish, Flaxseed AND Borage Oil capsules that contain Omega 3, 6 & 9, plus a bottle of Folic Acid tablets and a bottle of Vitamin E softgels.  And then there's the one-a-day vitamin with a little of everything.   

I fully understand the differences between water-soluble and fat-soluble vitamins, but have a question or two.  Any information you can give would be helpful and greatly appreciated!

I lost my health insurance a few years ago so my family doctor and I have been dealing with my previously diagnosed issues.  One thing he insists on is an inexpensive multi-phasic blood test sponsored by a local community hospital twice a year, so that we can keep an eye on any developments.  My next one is coming up on August 21.  Once he has the results in hand I'll make an appointment to talk about any changes in treatment.  In the meantime...

I am thinking about paying an extra $20 to have my Vitamin B-12 level checked.  I understand with age sometimes there's a difficulty in absorption of some nutrients, let alone the medications I take to control the reflux.  Should I hold off on taking any extra B vitamins until after the test?  I am a little concerned if there might come a time where I can't afford extra things like vitamins and nutritional supplements; would it be advantageous to know IF I have a problem with absorption?

My other question is, IS there a recommendation for what to take and how much in terms of vitamins and other nutritional supplements for someone with my unique-to-me set of health issues?  The gastric reflux started in 1980 (the H.pylori test was negative); the leaky gut and colitis issues from 10 years ago...

This is obviously not a simple question and cannot be answered with a simple reply.  If you need comprehensive advice I would suggest you obtain it by an online telemedicine physician such as myself (www.saleeby.net) or another off the web sites www.acam.org or www.lef.org.  In a nutshell however you may want to try a comprehensive balanced formula of MVM + n-3FA and other compounds in a "single" formula such as the Longevity Pack (www.vitasanus.com)  That way you get an engineered and well balanced formula that does not lend itself to mismatching or overutilizations of certain components.  Also there is a very well researched dietary supplement (meal replacement) that can be used as a substitute for "dietary supplement pills"... called Shakeology.  You will have to acquire it via a BeachBody coach www.shakeology.com is the site for this supplement.
