I've been having this item for years and have never known what it is. I have attached two images of it, it measures 7 1/4 inches high and 2 1/2 inches wide, the spool has the number 4391 on the bottom printed in ink.

Answer
This is most likely a homemade "knock down" doll.

It was made to be knocked down by throwing a ball at it, similar to the games played at fairs and carnivals.

The maker used an industrial thread spool for the base. The number has no mening to the doll.
