General Surgery/Bleeding Peptic Ulcer

Would a person who has a severely bleeding perforated ulcer require immediate emergency surgery? If so, what type of surgery would he require? If not, what would the treatment plan be? The scenario is that a person has an untreated ulcer, which has been slowly bleeding for awhile, that gets so bad he starts to vomit blood. I'm assuming he would need a blood transfusion too, right?

I really appreciate your help!

Dear JD,

A bleeding ulcer can be a medical emergency based on a few reasons. The two main reasons are gastric perforation and blood loss. An ulcer is an erosion in the lining of the stomach. Since it is now a bleeding ulcer, it has eroded down to the level of the blood vessels. A gastroenterologist is the best person to properly diagnose this. They are able to use a scope with a fiber optic camera at the end to visualize the damage. I recommend not putting this off. This can also be a sign of cancer. The earlier it is diagnosed, the better.
