It really depends somewhat on the construction of the sentence; for example, you would write, "These are small- to-medium sized companies," but you would write, "I take this in a medium size," etc. However, in most instances (I'd say 89%, just offhand), you'd use "sized" as the participle here.
"Medium-sized portion," "Medium-sized drink," "Medium-sized shirt," "Medium- or king-sized bed," etc.
