I have received the following address for sending a package to Greece.

goumenissa n.kilkis
61300
Greece

I don't see how this fits the USPS format of:

Name:
Street or PO Box:
Town/city, State/province, postal code
Country

How should I fill in the form.  I don't see a street address!

Many addresses in Greece do not mention a street address.

I would send the package as you have written it on your question, the zip code (61300) is the village/town and they will likely know the person you are sending this to or the person collects mail at the post office there.

It should be OK
