I have spell of almost passing out, I have had an angiogram and all is clear, I have pinpointed that eating bread makes things worse. My blood sugar seems to be fine, BP is in acceptable ranges. I have actually passed out twice. I have pinpointed the fact that most often it is after a meal or after drinking coffee.

Dear Paul,

More info needed.  How old are you?  When were the spells?  Angiogram of what? What do you mean almost?  What exactly happened?  Did you almost pass out only standing up, or sitting and lying? Any premonitory symptoms like dizziness, breathing hard, tingling or fingers or toes or mouth?  Has anyone watched you as you passed out?  If so, what did they observe?  Were you pale and sweating, breathing hard?  Any jerking of face, arms or legs?

Answer all those questions and maybe we can start figuring out what's the matter.  It's 10 pm here and I'm going to bed, so probably won't answer till tomorrow.
