Question
My friend is a male 53 years with hypertension on Tenochlor 100 once Losar-A twice He has a family history of diabetes and his mother died of diabetic coma at the young age of 59 years. Last Dec 2008, he had his blood tests checked in which other blood reports were normal but his fasting blood sugar was 112. His dr advised for OGTT and he was found to have Impaired Gluocse tolerance. However his dr advised for HbA1c which was 7.1 ( FBS 112-PPBS-100) in Dec 2008 Again repeat HbA1c was 7.7 ( FBS 116-PPBS 112) and in the last week it was repeated which revelaed HbA1c 8.20 ( FBS 116 PPBS 128). My question is why HbA1c is high and FBS-PPBS are withing normal range ? Secondly why has fasting BS is higher than Post Prandial. Thridly, dr suggestes Glycomet 500 as pre diabetic. Kindly guide whether my friend is diabetic or lifestyle changes can improve his sugar. He has as yet not strtaed the drug Glycomet Thanx

Answer
1. There is a possibility of fluctuations in his sugar levels during the day. That would probably explain the normal FBS-PPBS and high HbA1C.
2. His sugars are probably metabolised faster. that could be the reason for his lower PPBS.
3. Your friend is definitely prediabetic. I would suggest he improves his lifestyle and repeat his levels after 2 mths. Glycomet could be considered if his levels are persistently high.
