Question
Hello Dr Roseff,

I am currently 26 years old (soon to be 27) and diagnosed with unexpalined infertility.  I had surgery to remove rectal endometriosis last September and scar tissue removal surgery in March of this year.  My docs all say that was not necessarily preventing me from getting pregnant so I just assume that the whole synergy of the conception process is off between me and my husband.  I had one failed IUI prior to the laparotomy.
My doctor is now recommending more IUI's and next IVF which I plan to start next year if insurance is welcoming.  From my reading it seems that IUI never works and IVF barely works for unexplained infertility patients.  In other words your third IVF is most likely to work if anything is ever going to happen.  I have however noticed that these statistics are all referenced as experiences from women in their thirties.  I want to know if their is an advantage to IVF in your twenties for unexplained's?  Just trying to see if their is any light in this dark tunnel.

Hi Angela,

The success rate of IUI or IVF depends on LOTS of factors, including (and not limited to) the age of the female, the quality of her eggs, and the expertise of each, individual fertility program.

If someone has a history of endometriosis severe enough to invade the rectum, she usually (not always) has pelvic endometriosis.  Since endo is something that can lower the odds of conception (and therefore cause fertility problems), a woman with endo is not classified as "unexplained" in her infertility diagnosis.  I hope this makes sense.

IUI is generally highly successful in couples with unexplained infertility, as is IVF.  My average patient (especially in your age group) conceives on her FIRST IVF cycle.  Again, you need to check success rates at the center you're considering....  As an example, we post ours on our website at www.reproendo.com/ivfsuccess.htm

By the way, if your husband hasn't had sperm DNA fragmentation testing done, and IF it's abnormal, then that can be an infertility factor (and not unexplained).
