Infertility/Fertility - secondary infertility on clomid

Expert: Scott Roseff, MD, FACOG - 10/25/2009

Question
Hi, I have two children (we were trying for 6 months for the first, 1 month for the second) and my husband (31 years old) and I (31) are trying for a third baby.  We have been trying for 6 months, but I had been charting my ovulation for 3 months prior to trying.  I was not getting positive home ovulation tests.  My OB/GYN put me on 50 mg Clomid days 5-8.  It has been 6 months of trying and I have gotten positive home ovulation tests each month.  I have only gotten positive ovulation tests on Clomid.  Is it possible not to ovulate but still regularly get your period?  I had an HSG which was normal, and my husband did a SA with a "slight" morphology issue (1% were abnormal).  I was referred to a infertility clinic and have just started 50mg Clomid twice a day for days 3-8.  We are scheduled to undergo IUI this month.  My husband's SA will be performed again with this procedure to see if his morphology issue is consistent.  I am worried that I could be on Clomid too long or that it is depleting my lining.  All of my bloodwork results have been normal.  Is the IUI the right thing for us to do?  Thank you!

Answer
Hi,
Most women who don't have a positive ovulation predictor kit (but do have regular menstrual cycles) are ovulating (despite not having an LH surge on the kit).  If a woman ovulates regularly and normally, and if she takes Clomid anyway, it may PREVENT her from getting pregnant or cause a miscarriage.  The package insert for Clomid states it's a drug that is taken when a woman does NOT ovulate....

IF she needs Clomid, and IF ovulation occurs at one pill per day, then increasing to two pills daily has generally no benefit.  The package insert clearly states that once the proper/lowest effective dose to establish ovulation is reached, there's generally no advantage to increasing the dosage therafter....

Finally, IF someone needs two pills daily to ovulate, they're taken together (and NOT twice daily)....

Hope you find a good, Board Certified Reproductive Endocrinologist who will take great care of you!!!

I hope this helps,
