Answer
1.Question:
Should a Muslim utter the intention (niyyah) when he starts to do an act of worship, such as saying, “I intend to do wudoo’”, “I intend to pray”, “I intend to fast” and so on?

Yes but not necessary-it's all in your heart.

2.should a muslim had to receite everything what imam is reading during Jamat prayer.
NO

3.how we can pary the missed rakat when offering prayer in jamat.( please be more specific here like if i miss 1 rakat , 2 rakat ,3 rakat and for all 5 times)
2) For every rakaat you will read surah fatiha, for example If a late comer missed three Rak’ats of Zuhr Salat, he will recite Surah al-Fatiha with another Surah in each of the first two Rak’ats. In the last Rak’at, only Surah al-Fatiha will be recited. If you missed the ruku of the fourth rakat you will pray as if you are praying from the beginning.
3) if you caught the fourth rakaat then you will make up 3 rakaat, and on the third you will only read fatiha.
4) if you caught the last rakaat of zhur, then when the imam gives salam, you will stand and this first rakaat on your own is the second rakaat overall, as you prayed 1 rakaat with the imaam, so after this rakaat you will perform middle sitting, then in your own second rakaat (overall 3rd rakaat) you will not sit but stand straight away, then the last rakaat you will perform the final sitting.
