Laser Eye Surgery - Keratitis post-PRK

Expert: Dr Anand Shroff, MD, FICS - 2/28/2008

Question
Dr. Shroff,

Thank you for your availability. I am six weeks post PRK in both eyes. I started out -5.75 OS and -6.00 OD. Five days post-op, my left eye became painful and red and I was reduced to hand movement vision. I called my doctor and was told to come in the following day. He diagnosed keratitis (unsure whether infected or not) and placed me on round the clock steroids and antibiotics. The keratitis resolved and the epithelium in my left eye closed. Subsequent examinations showed a shrinking circular area of epithelial and anterior stromal haze. Currently, my right eye is 20/20 with some fluctuation but my left eye is blurry with ghosting with dramatic distortions of light sources (can't really describe them). I can see the small gray/white spot in my eye (about 1-2 mm) in the mirror. I am about 20/80 and 20/60 corrected (lost two lines of BCVA) My questions are : In your experience, do these type of complications resolve completely (or permanent central scar), will they result in irregular astigmatism and will I need addition surgery. Thanks in advance.

Answer
Dear Robert,

This is rather a difficult question to answer in specific to your eye since it involves us examinaing the same for the opinion. In the absence of which I cannot comment, except in general.

If the treatment of the keratitis is done properly, the prognosis is usually good.

With a good combination of eye drops (maybe steroid and antibiotics as suggested to you) with vigilanmt checks and regular examinations, you should be fine.

You can keep us posted on your progress.

All the best!

Hope this helps,
Regards,
Dr Anand Shroff, MS (OPHTH), FICS
