Hello Dr. Anand:
My mother has lost vision in one eye.
1. Doctors says that this happened due to rejection of the transplanted cornea by her body. Can that happen or is there some other cause for this?
2. Now she has been detected with cataract and keratoconus, can the two occur together? If yes, what according to you is the best mode of treatment?
3. One of the doctors' she consulted suggested that she takes C3R treatment, how far has this treatment been successful?
My mother has lost her vision in one eye, we are worried about the other, please help.

Dear Vandana,

Thank you for your e mail

1. Yes, a transplanted cornea can be rejeceted due to many reasons.The cause to be determined is difficult. However systemic illnesses like diabetes, etc can be a reason. It can be rejected if the graft was also not of good quality. You treating doctor will be able to narrow down the reasons for this rejection.

2. Cataract occurs in the lens of the eye. The natural lens gets cloudy due to degeneration. The only treatment for this is surgical with modern technology not requiring stitches (Phacoemulsification). There are different lens implants that one can use which is discussed again with the treating eye doctor as the choice of implant depends on an individual's lifestyle and eye condition.

Keratoconus is an abnormal cornea. The cornea is like the window of the eye (imagine the eye to be a closed room). The lens is somewhere behind this window. Hence, the two are anatomically not linked that one condition leads to another. By which I mean that cataract and keratoconus are independent in their occurrence, although may have been detected in the same eye.

Both need attention.

In keratoconus if the cornea is not too thin (more than 400 microns) and fits in the criteria for C3R treatment- Corneal Collagen Crosslinking with Riboflavin, then this would seem a suitable treatment.

I presume that this has been suggested in the non- transplanted eye?

I have been performing C3R for past year with excellent results and was one of the first to use this procedure in India. This treatment is very beneficial provided it is done on the correct patient. In our past years experience, no case has ever failed to respond.
