Answer 1. Yes, we believe in the whole bible. Quoted from Article of Faith number 8 "We believe the Bible to be the word of God as far as it is translated correctly" http://scriptures.lds.org/en/a_of_f/1
2. I don't think that we have an official opinion on this subject. I am sure that various people (mostly prophets) wrote the Bible. Traditionally, Moses is credited with writing the first five books.
3. Yes, there is a literal Heaven and Hell. Although, we believe that Heaven and Hell are not single places. There are degrees or levels for the righteous and well as the unrighteous.

4. Yes, we believe in the Trinity but not the traditional view from the Nicene Creed. We believe that they are separate distinct individuals.
