I always placed that as a Nuero spiked crossed memory, perhaps he was thinking back on a birthday gift his brother, or friend gave him also symbolizing the upcoming fat embalism that killed him. As he told his father,the general, he was just still tired, as he was so pumped with medications, hard to get comfortable.

I have been watching MASH since it was an original show, I also watch it 4 times daily in reruns, I can tell when something has been cut from a show (as it was on FX much more frequently than it has been on Hallmark).
