I think the pink shirt came along after the Season 7 episode "Peace on Us," where Hawkeye interrupted the peace talks. In that ep, he complained to BJ that everything in the camp was green (including their clothing) and for a change, he wanted to see some different colors. While I don't remember any dialogue that later connected that conversation to the pink shirts, that's what my interpretation was.

I think the pink shirt is actually one faded red thermal underwear shirt that softens in tone over the length of the program.

Actually, BJ's pink long john shirt is a remnant, left over from episode 702. At the end of the episode, everyone in the camp has dyed their hair and clothing red. They have a big party to honor Hawkeye, who went and crashed the failing Peace Talks. When he gets back they dyed his hair and surgical gown red, too. Though most of the things came out looking more pink than red. We see more and more of BJ's "pink" shirt from that point on.
