These are all commodity items that have a lot a competition and little profit. They are usually purchased through distributors like PSS who can provide all of a doctor's or clinic's needs with online or telephone ordering and quick delivery. Hospitals are just as bad.
My thought is that a small operation must have some special contacts or relationships with the potential customers or provide very specialuized supplies that require some expertise or support. That is the only way to differentiate yourself from the larger organizations.
I sugget you contact some local supply companies outside of your area to see if they will share some information as to their challenges and or success.
