QUESTION: De nuevo yo.... dónde realiza el trámite de cancelación de visa??? y me podría sugerir algún medio económico para solicitar actas de ancimiento americanas apostilladas del estado de Arizona... Gracias!!!

En la misma oficina en donde realizó el trámite para obtenerla o en cualquier Consulado de los EUA. Contacto: http://Mexico.USembassy.gov
Y las actas sólo pueden adquirirse con seguridad por medio también del propio Consulado o Embajada de los EUA.

QUESTION: Confirmando una de las respuestas que amablemente me dió... No hay problema que en mi acta de nacimiento americana aprezca oficialemnte el apellido paterno y en mi caso utilice paterno y materno en mi SSN, ID y pasaporte???

No, no tiene por qué haberlo. El problema sería que hubiese un cambio en el orden de los apellidos o estuvieran mal escritos o mal colocados en los documentos, por lo demás, no hay ningún problema.

QUESTION: En caso de olvidar entregar el permiso que dan como turista a la salida de los Estados Unidos, de qué manera se puede regresar para que registren que efectivamente la persona salió de territorio americano???

En el supuesto de que eso sucediera, puede acudirse ante el Consulado para reportar la situación, a la brevedad y explicando que fue una omisión involuntaria, pero que se desea mantenerse al corriente en todo. Hacer las cosas pronto y voluntariamente es meritorio.
