you could possibley be missing one of the washers under clutch hub wich are about an 1/8 of an inch thick.

We had the same problem. I think it is because of a axial freeplay in the transmission shaft due to wear. Rather than diassembling the whole engine to check up the tranny I did inserted a 1.5 mm thick shim behind the bearing of the pressure plate.
