This is cause by an underscore in the last line of the chrome.manifest file
(which is normally in this folder: C:\Program Files\Mozilla Firefox\extensions\{CAFEEFAC-0016-0000-0000-ABCDEFFEDCBA})
where it reads chrome/locale/zh_TW/ffjcext/
and should read:

chrome/locale/zh-TW/ffjcext/

edit it using notepad, save it and the bar should go.

That actually worked. that damned menuitem bar has been a scourge to my browsing.

I have the same problem, and it was also immediately after I ran a virus scan and deleted some files. I haven't added any extensions in some time. I've gone through a couple of debugging tutorials to no avail. Any help would be really appreciated. Thanks!
