Muslim Teens/Am I lost?

Aasalam Waleikum wa Rehmatullahi wa Barakatu brother.
I was a Christian but now I am a Muslim. I live with Christians and my whole family is Christian. I use to do lot of bad things when I was a Christian, and little by little I left everything for Allah and mysoul. I go to mosque atleast once a day, miss it rarely but pray alone and sometimes 3 or more times with Jammat. Even after doing everything for Allah Subhana wa Tala am facing symptoms of lung cancer because I use to smoke. Am just scared if I'll end up in hell fire and graves will squeeze me like anything? I say in the name of Allah I can say, more than 70% or 80% of the Jammat I have attended, I was on the first row. Even in Eid, I promised to Almighty that I will never complain him anything and even now I don't, because he is the All knower and he does best. Allah o Akbar. I can't go to doctors because I don't have money to give them or they will help me with less cost. Believe me am crying in my heart at every moment that what will happen to me.
On the other side am happy to meet Allah but the question is will he speak to me? What if he dosen't?
I know Almighty forgives everything, even if it is Blasphemy but the thing is we should be alive to ask for forgiveness. In my every dua almost I ask for forgiveness and hidayah for my parents. I made them understand a small thing about Christianity and they get angry saying "do you know better than us"?. I don't live in Muslim environment. So I don't have anyone to comfort me when ever I feel depressed and I keep crying most of the time in my heart. That makes me weak and think that Allah dosen't love me anymore. I know am wrong but am really weak to face this situation. I just do want Jannah but I want the best of Jannahs closer to our Prophet Muhmmad(SAWS).
Now the question comes to my head that Can I atleast enter Jannah?
Thank you in advance for your answer brother. I will pray that you will recieve one of the best Jannahs. Ameen

Dear Brother,

Relax !! You are going to Paradise.

Quran [20:82] And surely, I am indeed Forgiving to him who repents, believes (in My Oneness, and associates none in worship with Me) and does righteous good deeds, and then remains constant in doing them, (till his death).

Quran [5:9] To those who believe and do deeds of righteousness has Allah promised forgiveness and a great reward.

Quran [4:110] If anyone does evil or wrongs his own soul but afterwards seeks Allah's forgiveness, he will find Allah Oft-Forgiving, Most Merciful.

As for the disease try as per below guidelines and you will surely be cured.

Then one of our men went along with her though we did not think that he knew any such treatment. But he treated the chief by reciting something, and the sick man recovered whereupon he gave him thirty sheep and gave us milk to drink (as a reward). When he returned, we asked our friend, "Did you know how to treat with the recitation of something?" He said, "No, but I treated him only with the recitation of the Mother of the Book (i.e., Al-Fatiha)." We said, "Do not say anything (about it) till we reach or ask the Prophet so when we reached Medina, we mentioned that to the Prophet (in order to know whether the sheep which we had taken were lawful to take or not). The Prophet said, "How did he come to know that it (Al-Fatiha) could be used for treatment? Distribute your reward and assign for me one share thereof as well."
