Negotiating Disputes - Problem with credit card company's mistakes

Question
I am being left to deal with a huge mess due to mistakes made by two separate banks regarding a balance transfer I requested. Bank "A" accidentally paid Bank "B" twice because they wrote down the acct# incorrectly. When I noticed the error I called both institutions to have it corrected. Bank "B" was asked to refund the money to Bank "A" immediately and said they would do so. Instead they sent a refund check to a third party which was never received. Bank "B" was notified of the missing check and again was requested(and ordered under no uncertain terms)to cancel the origional refund and remit payment back to Bank "A" who was the rightful recipient. Bank "B" was told not to issue a check to the account holder, although they fought to have that authorization they were ordered to send the payment back to Bank "A" and said that they would. 7 months and almost 100 phone hours later both Banks are refusing to claim responsibility for their errors, holding me responsible to not only spend more of my time resolving their screw-ups but to be financially liable for their origional mistake. I didn't cause their mistakes and have tried in vain to resolve them but at this point I feel that I am not able to do more and that I hold no responsibility, financial or time wise to either of them.
Bank "A" is charging $3,184 (unauthorized by me)
Bank "B" says they sent out a refund to the third party whom I have no contact with.
What should I do?

How ridiculous this entire situation is.  I call this customer no service.

Contact The Office of the Comptroller of the Currency and advise them of the situation.  Request they get involved.
