Dear Dr Shah
Thank you  for your answer to my question on the above subject on 5 May.  I have also felt that we need to start looking at dialysis options (his serum creatinine is 300umol/l)but my husbands Nephrologist says he is trying to avoid dialysis and does not discuss it with us. I, as a lay person, do not see how this will be possible for more than a year or so.
I also wonder if you could tell me why his blood pressure fluctuates very greatly, from very low to very high, even when taken at the same time each day.  Does his kidney cause this? His haemoglobin is 11.6. When will he need treatment for this? His platelet count is 101. Is this a problem? I am just worried because his Nephrologist seems so casual about everything where to me he should be more worried. Thank you for your time.

Thanks for asking my opinion on "Allexperts".

I agree that you should start planning about various renal replacement therapy options with your Nephrologist. Please talk to him again and ask him to spend some time with you regarding this.

Your husband's Hemoglobin is within the recommended range of 11 and 12 gm/dl.

Unfortunately, I cannot hazard a guess as to the cause of his blood pressure fluctuations without actually examining him. BP fluctuations are not good for preservation of kidney function and must be controlled with appropriate diet and medications.
