hello dr. shah, i have a real problem with weight gain. im pretty sure my kidney disease is caused from 22 years of lithium for bipolar disorder. my belly feels like its full of water. i know kidney disease can cause fluid retention. what can i do about this. do i take a diuretic and what kind of weight loss method do i use. thanks.

my gfr is 58 and it should be 61 ml/min/1.73m2 or above

You have not mentioned your kidney function results. What is your S. Creatinine value and your estimated GFR.

The most common complication of chronic lithium therapy is nephrogenic diabetes insipidus.This means that the patient will have large amounts of dilute urine and will become dehydrated - diametrically opposite to what you are describing.

You need to be evaluated for your volume overload status with a proper history, physical examination and relevant lab investigations. Unfortunately, I cannot do this over the internet.
