Nephrology/Elevated Serum Creatinine


Dear Dr,I am 65 yr old male with HTN, Hyperlipidemia and subclinical Hypothyroidism, all moderate levels and under medication like 25 mg Metoprolol BD,5 mg Statin HS and 100mg Thyroxin OD. Strangely, in the year 2008,during annual meds,my S.Creatinine was found  elevated to 1.5 (ref range 0.8 to 1.4) from 1.0  till the previous year.This has remained more or less at that level even now after 8 years. Doctor, is this a normal progression of CKD ? I am asymptomatic otherwise and generally fit and active. My other blood works are within limits. Recent test showed no signs of Retinopathy too. Under the circumstances Nephrologist keeps observing the S.Cr values at 6 monthly intervals. However my Hb count has slowly increased from around 15.0 to 16.4 (latest) during this period. Has this level of Hb any cause of concern in my case ?

If I were your doctor, I would order an ultrasound scan to look at the size and echogenicity of the kidneys. Also, I would discuss the need for a kidney biopsy to find out the cause of the elevated S.Creatinine.

Elevation of Hb level also needs to be investigated. I would rule out polycythemia. Please discuss these points with your Nephrologist.
