ok so the ruiner can mean several things, an addiction, a girl, any thing that can fuck you over and make you feel like shit....
it really seems to be more about addiction though
so the ruiner (a drug) infects every thing you know and distorts your reality..
the world you live in when you’re an addict is not a very pretty one its full of lies, death and hatred
about the how'd you get big?
it really is him over looking his situation and wondering how this addiction could have become so enormous that you cant handle it, it over comes you in tilde waves ... as an addiction starts out you think you can controls it and here, he is stating that this thing has become to much..it now controls his life and there is no escaping

the same thing just how has this thing over powered me..its self explanatory "howd you get so strong"

is how far has this gone. he thought he had it under controls but now he can see that this thing is now interwoven into every part of his body and that this was a huge mistake..he docent know how far down it runs but he knows that it will be the hardest thing he will every have to do. to the point that it might kill him to try and clime back out of the hole he has dug.
and usually when he says "this thing" he is usually referring to a drug or addiction
as for the end "nothing can stop me now" its kind of like denial...trying to convince him self that he is ok but in reality nothing is right
