On my 2000 Nissan Altima, It idled rough but on acceleration it was perfect (kinda)  After changing all the typical items. MAF, O2 sensor, plugs etc.  I had NO change whatsoever.. I finally just took off the EGR and cleaned it and the EGR sensor (both located back center of motor) put it back together after a new gasket (make sure you clean off the old material on both ends) It ran absolutely perfect!  Make sure you remover the sensor next to the EGR first (14mm wrench) then remover the EGR valve (12mm wrench less than 6" long so it won't hit the firewall).  Over all I spent almost $500 doing it myself.  When all it needed was a clean EGR, and the price of $1.43 gasket!  I used brake parts cleaner on the sensor and EGR.
