Experience
Currently I am working as a Professor of Obstetrics gynecology, Menofyia University, Egypt. Since 1987, I worked in many university and private hospitals with a high load of pregnant population averaging 7-9,000 deliveries/year. This exposed me to all problems associated with pregnancy, labor and delivery. I had my doctorate degree in Obstetrics and Gynecology in 1996 in the use of Doppler ultrasound in high risk pregnancies.

Question
I am 6 weeks and 5 days pregnant.  Yesterday i started spotting red which only happened in the early AM then went away and since i was supposed to have an appointment today anyway i went in for an ultrasound yesterday and the Dr. said my cervix was fine and then we did an ulrasound and saw the heartbeat and he said there was nothing suspicious about my uterus or cervix so no bleeding or anything weird.  I spotted again today and it was a little more but also only in the early AM by now it is totally gone.  I am on progesterone suppositories 200mg 2x a day and i know that can cause irritation so i am wondering if you think that's all it is.  the blood usually comes out with the excess progesterone.  I haven't had any cramping at all and if after i spotted yesterday my cervix was closed and we saw the heartbeat does that mean it's probably not miscarriage related and the bleeding today is for the same thing?  Sometimes i feel a pinchy feeling or it feels like i have a scratch on my cervix or deep in my vagina (usually this is when the bleeding is occuring) something like that so maybe it is just irritated.  i use the suppositories 2x a day and only have this problem in the morning but that's when i am standing up becuase i am at work, at night i am usually on the couch or in bed.  thank you for your help.

Dear Kristin

This is a case of threatened abortion with a 50% chance to continue or to abort. The most importnat line in the management is physical and mental rest. You should spend most of the time in bed and avoid household work, heavy lifting, excercise, sex and all activities which require physical effort. Bed rest should continue till after bleeding stops for 48 hours. This is waht i do with my patients. I understand that this can have some financial effects because u will be off work for a few days but as i mentioned earlier, bed rest is the first line of treatment. Threatened abortion is diagnosed in a pregnant lady in her first trimester when the cervix is closed with a history of spotting and abdominal cramping.
