ObGyn/Pregnancy issues - Large subchronic hemorrhage

Expert: Cheryl Hetherington - 6/28/2009

Question
Hi,
I had a spontaneous abortion in nov 2008, and had a positive pregnancy test on the 26th of May. I got an hcg done on the 28h of may and the blood count was 1390. I travelled to India after that. Everything was going fine, until Ihad some dirty brown discharge. I went to the gynac here in India, and they did a TVS. They put me at 6 weeks 3 days, with a CRL measuring .76 cm and a yolk sac, etc, but no fetal cardiac activity. I did an hcg test and it was 19700
I was told its a 50/50 chance, and to come back for an u/s after 10 days.
I called my dr in the US and they said everything was fine and not to worry. I had slight spotting again on thursday night and went in for an U/S on friday, which was exactly after 11 days from the last tvs
they could not see anything in the u/s and did a tvs again.
They diagnosed it with a GS measuring up to 6 weeks 2 days,no yolk sac or fetal pole and a large hetroechoic area of subchronic hemorrhage and a case of missed abortion.
My quest is, is there even a slightest of chances? (I know its silly to have hope). I havent had any bleeding so far.

My next ques is if I wait for the bleeding to start and the body to expel it out naturally will it cause any harm or infection in me. the dr has suggested a d&c. I am just confused, as I have not started bleeding, what should I be expecting .
Your expertise would be of great help.
Thanks so much in advance.

Answer
I am very sorry for your loss. A D&C would be the best and the safest course of action. There is a possibility that you could develop an infection or other complications by trying to have a natural spontaneous abortion. At this point, given your history and the situation, I would tend to agree with their diagnosis.

You will certainly be in my prayers, and I wish you the best of luck in the future. Please do not hesitate to contact me if you need anything further.

Best Wishes,
Cheryl A. Hetherington
