Ophthalmology & Optometry/Slab off prescription transfer

What I suspect is the first company (EN) did not account for the slab off based on my follow-up question to them after the optometrist would not give permission to (AE) for the slab off.

There's no doubt the slab off is warranted but I will try getting the prescription.  (I'm old enough to know better.)   

I'll attempt to grab the prescription from EN this morning in person since I'll be in the area.
