Ophthalmology & Optometry/Cataract surgery following lasik

I had cataract surgery about 8 years following lasik.  The Opthalmologist had my records from the facility that performed the lasik.  He did measurements 5 different ways.  The surgery was successful but the results were not.  I now have 20/400 vision.
