Oral Surgery - oral surgeon said I was too apprehensive

Expert: Joel S. Teig, DMD, FABOMS - 11/6/2009

Question
I was scheduled to have five upper jaw teeth pulled, but first I met with a student dentist while he read all of the things that could happen while having my teeth extracted such as a broken jaw which he said is rare, then he told me how to care for my gums afterwards. I asked him if the student who would be pulling my teeth aspirated the needle and he said yes. Being on an MAO Inhibitor, I wanted to make sure.  They had already talked to my doctor and he had agreed to let them use epinephrine. I had already some teeth worked on fillings, deep cleanings with it and there had been no problem.  Oh yes, I mentioned to the student that years ago I had panic attacks which is why I was on the MAO Inhibitor. He told me that his sister had them and could not leave her house. Then, he had me sign the permission slip and left. I thought I would be taken back to have my teeth extracted, however the student dentist came back and the oral surgeon came and stood right in front of me and said, "Because of the health problems you have had, we have decided that you should go to an oral surgeon. " Since January, I have been having work done in preparation of getting dentures on top and a partial on the bottom. I didn't know what to think. so, I just left. I phoned the patient advocate later and ask if she knew why I was told to go to an oral surgeon and she said, it said on the form that I was too apprehensive. Now all of the work I have had in preparation for getting my dentures has been in vain. My regular student dentist who has worked on me all year told me if the oral surgeon wouldn't supervise the student while he pulled my teeth, then there was nothing they could do. My husband said I should pursue this, but I don't know what to do?  They have all of my xrays and records of all of the work I have had done and even an impression for my bottom partial. Is there any way I can get these things, I did pay for all of them and maybe I can find someone else to take over. I was not that apprehensive at all.  Do you have any idea what my options are. Do you think the dental school would give me my xrays and records? My husband is retired
so we no longer have dental insurance through his employer which is why I won't be going to an oral surgeon and I don't know what to do. Thank you.

Answer
Connie - I do not know for sure why the surgeon refused to let you have treatment at the school, but the use of a local anesthetic to make you numb should definitely not contain epinephrine, if you are taking an MAO inhibitor.  It is potentially dangerous and can precipitate a severe hypertensive episode.  You can still have a local anesthetic, but it cannot have epinephrine.

The xrays, modeals and records are yours and you have the right to have them.  You should see a surgeon for the xrays, but the surgeon can work with a student dentist to fabricate a denture after the extractions by a surgeon.

So go to the school and request your xrays.
