David, I have this exact same problem.  I am a young man and in good shape and out of the blue for no reason my ankle will have the worst sharp pain.  I will be sitting down or standing, it doesn't matter.  It lasts for about 5 seconds, and completely disappears.  It is a very intense pain.  Like you say, I don't even think about it because it is rare (like every 6 months or so).  Anyway, thanks for asking this question, and I will take the doc's advice.  If it were to happen while I am driving, it could be bad.

Hi, I too have the same symptoms. Sharp pain in the ankle for a few seconds then disappears. The frequency is increasing, about 6 months ago; it was once in 2 weeks. Now it occurs about 3 times a week.

Same issue: occasional severe but brief pain. I could be pain free for months and then one day, I get many episodes. My best guess is a pinched nerve or tendon, and that it has something to do with shoes I seldom wear, and the pain comes 1-3 days later, so it is difficult to make the exact connection.

I'm having this exact same issue and had two  x-rays done on it with nothing wrong found. I'm glad I found this question and that other people are having similar issues, but nobody has posted their findings or results of what they did and what kind of doctor they saw, or the process they went through to correct the issue (if they were able to correct it). Can someone please post this information? Was it a chiropractor you went and saw and did they fix it? I went to my normal doctor and they took x-rays, who then sent me to an orthopedics who took more x-rays, neither one found anything. Please let me know if someone had good results to stop this and the steps they did to correct this. I'm going crazy, had this off and on for a couple of years now. Thanks for any help someone can give.
