Pediatrics - Weight Problems with my 3 1/2 month old

Question My daughter Madison is 4 months old on November 7th. As it stands now she is 11 lbs 14.5 oz. she has been at this weight for 2 weeks now, has not gained an ounce. She is a breastfed baby, switching to formula because the Dr's feel my breast milk is not giving her what she needs to gain her weight. Madison was born 18 days early and weighed 5 lbs 13 oz. So she has gained over 6 pounds since birth, which I think is good. But I am worried as she has just upped and stopped. She eats great, she has BM every day and every diaper I change is wet. Any advice?
Answer maintain the weight monitoring every 15 days. follow a written feeding schedule or note down feedings for 2 weeks.
simple errors of weight may occur due to clothes, passage of stools full stomach etc.
not gaining weight needs to be proven for a significant period like say 2 weeks,despite increasing frequency of feeds.
