Question
Dr. Olsen,
    My 8 year old daughter has been diagnosed by her pediatrician and a pediatric gastroenterologist with IBS (constipation predominant) and GERD. She has always had lots of gas and indigestion symptoms since infancy. She has also always needed to eat small frequent meals with protein or she has dramatic mood swings.  She has "heat flashes" and splitting headaches sporadically. Her temperature is consistently on the low end at 97.8. I have Hashimoto's Thyroditis and my sister has thyoid problems also.  Recent blood chemistry profiles indicate high fasting glucose of 128, high alkaline phosphates 305, AST(SGOT)36. Low creatinine .5L and Mono Absolute .3L. Urinalysis normal and abdominal ultrasound normal. She recently had agrowth spurt but now weighs 3 pounds less. So she is losing weight unusually.
Is there probability that she has thyroid issues and if so what diagnostics and treatment are available?

Hi, Linda,

Has she had any thyroid studies?  I will assume she hasn't.  

The FBS is a bit high and I think probably needs to be repeated.  If she is heavy, then the idea of this being an initial sign of type 2 diabetes needs to be considered.  The other lab studies are normal for age and don't concern me.

Certainly she should have thyroid function tests done.  Her symptoms could be due to thyroid dysfunction and the family history is very strong.  A TSH and thyroid auto antibodies to rule out Hashimoto's could be done.

Talk to your pediatrician and see if these ideas make sense.
