Question
My Daughter born on 12.3.09 was detected with jaundice on the 3rd day of birth
(direct bilirubin:4, indirect: 8)birthwt being 2.64 kg born on the 40th week by
c section. after 15 days her birth wt was 2.39 kg with jaundice( direct/conjugated: 10,
unconjugated:0.8)After 3 days again blood report showed direct: 8, indirect: 0.92. after
that for 3 weeks the blood report showed little decrease in bilirubin latest on 17th
april being direct:6.23, indirect 0.95.Also liver enzymes were raised.  heida scan revealed no bile was coming out of
liver hence biliria attretia was suspected. but MRCP and USG(3 times) revealed that
biliary tract was not attretic (hence not biliria atresia). hence it was
concluded that she had intra hepatic cholastatis due to idiopathic neonatal hepatitis
since all other blood tests revealed no infections nor galectosemia or tyrosemia
(CRP, torch negative). At 1 and half months tentatively she started developing a
cystic development near the parotid region. FNAC revealed it was benign. Now her
age is 15 weeks and her wt:4.1 kg and the cyst is huge has developed 7-8 times the
original size. Developmental milestones are happening on time. Please tell us what we
should do. Are any other tests are required? jaundice is still there as is evident
from the eye skin and stool colour(though it is less pale than before). Please help us
and tell us what you think about our daughter's case and what treatment we should do.
She is currently on vitamins(oral) and vitamin K injection each week (2.5 mg). She is
drinking half NAN-1 and Half Nusobee cassein (just started to provide with MCT). She
sometimes has a little fever which goes on its own. Also previously she used to cry
just before urination but urine culture did not give any infection report. Hoping to
get an answer from you soon.Please tell us if any treatment is possible. She we make any
dietary replacements? What is the prognosis?

i have only one more suggestion, no further tests.. except a trial of intraoperative liver biopsy to confirm the biliary atresia.

all other investigations turn to be ambiguous and surgeon has to think of this is mind.

if it were biliary atresia, prognosis can eb better only if operated in first 3 months.

if its not biliary atresia, neonatal hepatitis is difficult to treat as there isnt anything that will do magic in a month. time cures most such infection over months.. if it is related to some infection, a amjor will settle in few months.

well i have no clue about teh neck/cheek swelling.. sne dme a pic of it if possible.
