Question Hi
I have an elderly friend (in her eighties) who was admitted to the hospital with sped up speech and strange bizarre behavior. She tested positive for methamphetamine in her urine toxicology but the serum test was negative. I believe the serum test was not done until the next day. Her hyper behaviors were calmer by the next day. This lady seems to have history of paranoia and mental illness and is reporting some very bizarre reports, one including people hiding yellow powder substances in her room. Could she be exposed to meth in some form yet pass a blood test?
Answer Following oral administration, peak methamphetamine concentrations are seen in 2.6-3.6 hours and the mean elimination half-life (i.e. the time taken for blood concentration to become half of peak level) varies between 2.1-15 hours in different individuals. Because of this, it may not be detected in blood the next day after taking the drug.
