Subject: drug reactions
Question Hello,
I am a 44 year old female who just recently had a hip revision done. I have epilepsy which I take 700 mg of CARBAMAZEPINE. When I was in the hospital I was given shots of WARFARIN(sorry do not know the dosage). Is there any known reactions to these two drugs? The reason I ask is because I had a case where my blood pressure went very low and I became unresponsive. The surgery was not successful and I had to have another revision. Again I had another incident where my blood pressure went low and I again became unresponsive. My epilepsy is very controlled. My seizures are only at night and are very short. I have not had any seizures for over a year. The reason I ask is again I must have the hip done again. My second surgery I was put on Lovenox shots for 30 days no problems. Can you substitute Lovenox and Warfarin? Thank you so much for your advice...
Answer Hi Debbie,
There is an interaction between carbamazepine and warfarin. However, the interaction does not cause low blood pressure. The anticoagulant effect of warfarin may be decreased when given in combination with carbamazepine. This is corrected by adjusting the dose. Lovenox can be substituted for warfarin. In fact in our hospital we have people stop warfaring 5 days prior to major surgery and have them take tinzaparin which is similar to Lovenox 5 days prior to surgery.
