Answer
I guess it's possible.  We derailed an engine 3 years ago going 30 km/hr.  Our excursion train has a diesel engine at each end.  One pulls us out about 12km and the other engine brings us back.  We run 3 trips each day.  On the last trip that day, the back engine which was just being towed had one wheel slip off the track.  We hit a siding and the engine fell completely on its side.  It was a combination of a wheel being slightly out, maybe a loose tie, plus a 35C day.  I was talking to a group and fortunately everyone was still seated so there were no injuries.  Exciting!
