Plastic Surgery - Pro-phylactic bilateral mastectomy and reconstruction

Expert: Alan M. Engler, MD, FACS - 11/7/2009

Question
Hi, I may be eligible for a skin-sparing mastectomy, to be preformed prophylactically, although there is extensive ADH in the right breast already.  I'm interested in implants, to avoid additional unnecessary surgery (as with flaps).  Have you heard of something called the Becker implant? How would I go about finding the best cancer and cosmetic surgeons for my procedures? It would be possible to travel for the procedure, although Washington DC area or other East Coast would be most convenient.  Thanks so very much.

Answer
Hi Suzanne,

A Becker implant is an expandable saline breast implant (ie, they leave in a valve for a few weeks so that you can have the size adjusted somewhat).

However, there are many issues that you'll need to think about - such as whether you should have a silicone gel or a saline implant to begin with.  Many people feel that a silicone implant provides a nicer result although that is clearly debatable.

The head of plastic surgery at (I think) Georgetown is a wonderful plastic surgeon named Scott Spear.  Contact his office and see where they would recommend that you go.

I hope that this helps, and good luck,

Dr. Engler
