Expertise
I am a certified plastic surgeon with special interest on aesthetic surgery, practising in Dubai, U.A.E. dealing with patients form diverse nationalities. You can address questions to me on body and facial aesthetic surgery (liposuction, breast surgery, nose corrections, eyelid surgery, fat injections, botox and fillers) as well as reconstructive surgery. I also have experise in the area of peripheral nerve surgery such as carpal tunnel syndrome and other nerve entrapments. I would especially welcome queries from net-surfers from this part of the world.

Experience
I have 17 years experience in reconstructive plastic surgery and over 12 years in aesthetic surgery. After qualifying as a general surgeon, I was fortunate to work in a unit that did complex reconstructive surgery and general surgery as well. This spurred my interest in plastic surgery as a career as the possibilities seemed to be amazing and limitless. This was the era when microsurgery and flaps captured our imagination. I further acquired formal training in Plastic surgery, burns, maxillofacial and hand surgery. this was followed by a training apprenticeship in aesthetic surgery. In my practice as a plastic surgeon, I do more aesthetic work now and am keen on developing minimal invasive surgery. I am a member of the Association of Plastic Surgeons of India, International Society of Aesthetic Plastic Surgeons, Royal college of Physicians and Surgeons, Glasgow, U.K.

Plastic Surgery - bladder/tummy tuck

Expert: Mohan Rangaswamy - 6/18/2005

Question
No I didn't have a congenital condition.  My surgeries started in 2002 with a hyster and they tied the bladder back up into place  My Obgyn thought my condition was related to the fact that during my first child birth I had pushed for 4 hours.  The muscles in the abd. are all stretched out and cannot bounce back.  I became pregnant again then I started having preterm labor during the 4th month of pregnancy...was put on bed rest because I was already dialted to 5cm.  About 6 months after the hyster, my bladder had prolapsed again...they tried the pessery(spelling not sure about) kegels (which have none0.  I had surgery again to tie up the bladder and rectum.  Here it is about 6 months after surgery and the symptoms of lower back pain and the "pulling" feeling are back.  I have noticed a small ball shape but the surgeon says that the bladder is still up.  They have also diagnosed me with Interstial Cystis...and treating with Elmiron...

I hope this helps.
Amy

Question -
I have had 3 surgeries to help my bladder from staying prolapsed.  I have heard that if I got a tummy tuck to tighten the abdominal muscles it could help hold the bladder in place.  Please help me out>

Answer -
Pls give me more details. Did you have a congenital condition of extrophy of bladder? Your exact problem is not clear from the query.

Answer
Bladder and rectal prolase with uterine descent are problems associated with pelvic floor muscles. this has nothing to do with abdominal muscle weakness per se. however if you have an abdominal apron hanging in front of the pubis and if the rectus [not rectum] muscles of the abdomen are separated i.e there is a midline bulge in the abdomen, then these can be corrected with a tummy tuck. Doing this "may" help the back pain if that was due to the heavy apron. One way of knowing this is to support the apron with your hands and see if that helps the back pain. I am not sure if you have the above mentioned problems with the abdominal wall.
Re: the pelvic floor repair since you have had it done twice, further surgery in that area is not recommended at this stage. You should concentrate on losing excess weight, general toning of the body, specific yoga exercises  for pelvic floor tightening and Kegel exercises. There is also a biofeedback system which your local physio departmetn may have. This can help you to learn to isolate the pelvic floor  muscles for exercising them.
