Question
My father was holding a car tow up for my brother to drive a car onto it. When the car drove up, it became too heavy & fell on my dad's foot. He was wearing flipflops. He then drove 9 hours with it continuing to bleed. We got him to go to the ER the following morning. The x-rays showed that the entire bone in his big toe was crushed. He also has a stress fracture in the top of his foot. His foot continues to bleed and he thinks that a piece of the crushed bone poked through his skin on the underside of the toe b/c there is a hole there.
They could not doing anything there (it was a smaller VA hospital) & referred him to the larger one in Pittsburgh for three days later.
We are wondering what can possibly be done to correct this. They had mentioned that it may need to be amputated. Is this a high risk?

Hi Marie,

This is considered a serious fracture with bone sticking out or protruding from a hole.  This is considered an open fracture and places the area high risk for infection.  With or without the bone protruding, a crush injury can easily end up in amputation due to the severe trauma to the area.

However, an attempt should certainly be made to save the toe.  The best option would be to get to a Wound Healing Center and see if they have a hyperbaric oxygen chamber.  Antibiotics are often important. The foot should be immobilized to decrease movement and additional trauma. I would recommend that he not put pressure on the foot.  Either use crutches or a wheel chair.

I would see a Podiatrist that deals with trauma and wounds. Certainly seek several opinions.  

Amputation is possible but there is certainly good reason to do anything possible before considering this option.  See a bunch of centers below.
